The printable symptom worksheet
The optional symptom worksheet builds a summary in your browser. Its entries are kept in the page's memory and are not submitted to this website, saved in browser storage by the worksheet, or added to a service request. Use Clear worksheet or reload the page to remove them. Copying and printing use your device's clipboard and print functions. You choose whether to share a copy with anyone. The blank PDF requires no signup.
Information the service-request form collects
The form collects the name, telephone number, ZIP code, selected dryer symptom, vent-exit information, optional message, consent choice, referring page, campaign labels when present, submission time, and limited security data needed to prevent abuse. Raw internet addresses are not stored with lead records.
How the information may be used
- Evaluate whether the request fits the guide's target area.
- Contact the person manually about the specific request and, only after identifying a provider and obtaining permission, share the approved information with that provider.
- Deliver and troubleshoot the request.
- Measure call and form performance without selling unrelated lists.
- Prevent spam, fraud, and misuse of the website.
Provider status, sharing, and compensation
No provider is currently assigned. Form submissions are not shared with an unnamed future contractor. The Local Lookout LLC will first identify a proposed provider and obtain permission before sharing a stored request.
If a provider relationship is activated, that provider may pay The Local Lookout LLC a fixed monthly marketing fee for access to eligible, permissioned inquiries. Homeowners do not pay this website for submitting a request.
Maps, hosting, and analytics
Embedded Google Maps content may receive connection, device, and interaction data under Google’s policies. OpenAI Sites hosts the application, and Cloudflare D1 stores service-request records. Resend delivers request-notification emails. Hosting infrastructure may keep security and delivery logs. This site does not currently use a public analytics service.
Call tracking
The published telephone number is provided through CallRail. CallRail processes caller number, call time, duration, routing details, and related technical data. The number currently routes directly to voicemail. Inbound-call recording and voicemail transcription are currently off, but a message deliberately left after the greeting is recorded and stored by CallRail for The Local Lookout LLC to review. If full-call recording or transcription is enabled later, this notice and the greeting will be updated before use, and callers will receive clear notice before recording or transcription begins.
The current form consent permits a manual response call about the specific request. It does not authorize automated or prerecorded calls, text messages, or unrelated marketing. SMS intake and automated text follow-up are not currently used.

Retention and choices
Identifying form fields have a 90-day retention target. A protected automatic cleanup is scheduled twice daily to clear expired names, phone numbers, ZIP codes, messages, and campaign labels. Cleanup also runs during subsequent intake activity. Scheduled runs can be delayed or fail, so deletion may occur after the target. Non-identifying operational records may remain for aggregate reporting and abuse prevention. This cleanup applies to the form database; it does not delete email, voicemail, or infrastructure backups.
To request access, correction, deletion, or a do-not-call notation, email the address below. Identity may need to be verified before a request is completed. Reasonable access controls are used, but no internet transmission or storage system can be guaranteed completely secure.
Copies in other services
Resend retains email and log data under its plan-specific policies. Delivered notification emails remain in the operator's mailbox until separately deleted. CallRail's standard policy retains communication records, including voicemail, for 25 months; some account and contact data can remain while the account is active, and deleted data can remain in backups for up to another 90 days. Contact us about deletion so we can address relevant copies and, where necessary, request removal from the service provider.
